Very simple troubleshooting software

We want a simple software for a factory that has multiple machines, each machine has Errors Check List. Usually an engineer passes around the machines to do the checkup and troubleshoots if there's an error he marks yes if no he marks no. Then after the paper work he has to enter it on the software and result will be either exported as PDF or Excel or can be emailed directly to the required department. So it's a very simple software let me list below the features required. 1. Add and Remove as much machines as wanted (e.g. M1, M2, M3) with name, type, model of each machine. So it will have 4 main fields (Name, Type, Model, Usage Purpose) 2. Add the Errors list per machine. Example M1 has Error 1, Error 2, Error 3 and M2 has EL1, EL2, EL3. So the fields here are attached per machine (Error Name, Error Description, Error Type) 3. 3rd feature is the troubleshoots paperwork entry. Example after the paperwork check-up the engineer goes back to his office, opens the troubleshoot tab and lists all machines then under each machine the list of error. Near that he has 2 checkboxes either Yes or No to mark based on his check-up result. 4. After Troubleshooting entry he has the option to list all the Yes's and No's each aside. Let's say first page it will list all the yes attached to error and machine and the second page it will list all the no attached to error and machine. 5. He has the option to export the results as PDF or Excel or he can directly send it email. Waiting your proposals kindly be clear and final.
