[Wordpress] Create a multi-page questionnaire using Contact form 7 or other

Before looking at this, i have to add that my site is NOT live. Im working on it via mamp on my local computer. I need someone who is familiar with contact form 7 and wordpress . if you've worked with the membership2 or buddypress plugin that would be advantageous. i am looking to get a questionnaire done that looks like the one on this website. [login to view URL](example screenshot attached below) . On the 2nd page, when you answer a question, the following question will appear via some css animation. Currently my site does this but there are some things i want u to change : 1. text for the radio buttons appear on the right of the radio button. right now its on the left. 2. list of radio buttons have to be arranged in a vertical form. like the one shown in the screenshot. 3. css animation for next question to appear 4. once all questions are answered, submit button appears. 5. for this project, just insert dummy questions. e.g what is your fav food? etc. i will change the questions to suit my needs on my own 6. add a small question mark beside the questions. when user hovers or clicks on it, a small window will appear that provides more information Lastly, if you have a better way of achieving all these (by using another plugin instead of contactform7) i am also open to it. I just need my questionnaire to work like in the example given. Thank you
