I understand that you need a web based application to maintain some records. I have gone through the attached images and understand that you need the same fields as you maintain the paper record. The user will fill all the record fields and generate a report.
You want to maintain 2 records. In the first record, some important fields are- Sr.no. , Description, Quantity, Heat tapped time, Quenching time, Remarks etc.
In melting report- S.No., Heat on time, Breakdowns, Max KW etc should be maintained.
Please let me know if you want to add some more fields on this reports.
