Need insta-post widget created for Wordpress

We have a live broadcast that is being done in UStream, which is embedded in a wordpress page. In order to be able to take questions, we want a Wordpress widget developed that does the following: 1. Allows people who are watching the broadcast to submit a question. 2. When the question is submitted, records the question in a database table for question history (question, asker's IP address, asker's name, and a timestamp). 3. The page will need to refresh the question area only (using jQuery ajax) once / second to display all new questions. (We are not worried about server load. Attendance won't be exceeding 50 people at once.) 4. Questions should be sorted in descending order (ORDER BY timestamp DESC). Additional information: When a new question is found from the ajax query, the existing questions should be moved down using jQuery slideDown(), and the new question should be shown a the top of the list using fadeIn(). This necessitates that each question be placed in it's own div inside the div that holds all the questions. We will provide a PSD mockup of the question box. Technical requirements: 1. This must be written as a wordpress plugin. We need to be able to add the Q&A section to any page using the [QNA] shortcode. 2. You MUST document the plugin using phpDocumentor. 3. You MUST use subversion for code version management. (We will provide the subversion server). 4. You MUST follow coding conventions as stated at [login to view URL] 6. This plugin will be developed on a development server, and then deployed to the production server using subversion. You'll have access to the development server, but you will NOT have direct access to the production server. 7. Any references to the domain should be relative references or be done using server variables. NO hard-coding of servers is allowed. 8. The table definitions for the database tables used in this plugin must be provided because the production server uses MySQL replication for redundnacy, and we'll need to add this to the server for you during roll out.
