Create Simple Website, User Forms and Email Templates

**Design simple website using ASP.NET 2.0 with SQL Server 2005 database. Website will consist of about 15 pages with a series of web based user forms to fill out. We will supply the content and the questions on the forms. The website will send out a few different emails to users based on different circumstances. This website will be an almost exact clone of other website. If you want to view the website or more details on the project, please let me know. As an "on-the-job trial" please create a one page form, which will be one module to this project. Capture contact information (name, company, address, phone, and email). Also put a Terms and Conditions scroll box on the page. The user will have to scroll to the bottom of the scroll window and also click an "I Agree to the Terms and Conditions" check box before submitting their information. Please bid this job as a fixed fee and also account for any incidentals that may arise while developing our project. We will give the final approval on each aspect of this project, such as GUI functionality or other modifications.**
