Sign Up forms on my website

Hello, im looking for a quality webprogrammer to fix my existing website. All you need to do is fix a sign-up form on my site where people can ask for further information. in addition to that simple form i want you to implement a second sign-up form a littler lower down the website. Those two forms need to be installed on 6-8 pages. after a person has filled out the sign up form i want him to end up at a thank you page! All emailleads have to come from my server and i need to know from which page the sign-up came so you have to put some additional text with each sign up form so i can track the sign up! this is a simple project and i want it to be finished in approx. 24-48 hours after i put the money in escrow! Price will be below 30$!
