Joomla Customised Comments Form

Hello, I have a joomla website and I need help with customising an extension. Nearest extension I have found is JA comments although there are many others that do similar job but not quite what I need. What I need: I need a comments extension for joomla that will sit at the bottom of each article. Instead of asking people to comment, I need that comments/feedback form to ask 3 questions i.e. rate it 1-10 (radio buttons), why? and how?. Once someone comments/leaves a feedback this should pop up in a module on the front page. Ideally only article writer should be able to reply to the feedback left (not essential, price depending). I was able to customise JA Comments extension to do this (almost) ;) I was able to edit the comments form to ask three questions and add a rating 1-10 but the form would only submit an answer to one of the questions. This may be a very simple fix or may not be ;) Hope the above make sense. I have a budget of up to £200 for this. Can this be achieved? Ideally I would need this to be compatible with Joomla 1.5 and 1.7 Many thanks
