I have a physical therapy practice and we currently use a paper form to gather patient information, and also to collect insurance coverage information from the patient's insurance company. I'm in need of a skilled freelancer who can create a sophisticated data entry workflow for my in-house staff, based on my paper form. Here’s what I’m looking for: Requirements: - Expertise in form development. - Some of the data collected will be free form (such as patient name), and other data will be from a list of radio buttons. - Ability to implement conditional logic within the form to control the next page and/or fields that will be displayed for data collection, based on data/choices of user. Also, the logic may cue the user to provide specific information to the patient. - Form will need to perform real-time calculations based on user inputs - Ensure data validation for some numeric values, and proper date formatting - Enable printing of a summary of the data collected, with corrective steps required by the user, based on the data collected - If you are proposing any web/cloud based form, the form must be HIPAA compliant (because it will include patient data). Skills & Experience: - Proven experience in form design and development - Strong understanding of client-side scripting - Proficiency in data validation techniques If this sounds like something within your wheelhouse, I invite you to place your bid. I can take your existing form and create a fantastic looking, user friendly Excel form utilizing drop downs, radio buttons, free forms, etc. I can lock down cells that shouldn't be edited, and create it where the user entering the date can simply "TAB" to their next field in the form (minimizing the need for mouse interaction). I will also have all of the collected date dropped into a tab that can easily be extracted to import into whatever client tracking software you may use. I am a born and raised American living in Alabama. Effective communication throughout the entire process should not be a problem.
