As you plan to use the intranet, I recommend you use MS Access as a central database to collect the data. If you use Excel, this allows you to work only on a personal station. Then you'll need to gather all files together and unite them manually. By this price, I expect that want to do MS Access database to enter data of the survey (Around 100 questions). If you don't want this solution and you want to use only Excel still. I'm ready to do the form in Excel for 7000 INR. But at the end of data entering you'll be required to unite the databases of separate operators manually.
