Design a customised form for registered users on a website

I want a form designed for our registered students to fill that contains the fields as contained in the attached file. The write up should limit the number of words to be entered to 30 characters. And after each student completes his/her form online the admin person should then be able to download all the entries either individually or all at once (if this is possible. The form should contain their photograph, personal details etc. Perhaps what is most important is that only registered students can have access to the area to fill the forms, to avoid hackers/robots invading the area. Our school site in done in Wordpress but the registered user area is in Moodle. You may design a new login area outside of moodle but a student must login first to complete the form.
