I want the format to come to the email in a pdf format, here are the links on how to set that up. [login to view URL] [login to view URL] [login to view URL] If you go to this page [login to view URL] and click on the General Liabilaty Application, also the General Liability form will need set up to look just like the one attached and when I get it as a PDF. it is already set up with Contact 7 forms but not as to come through as a pdf, the top 2 will need set up to look like that layout and I am sending the 2 forms with this so you can see them. Also if possible I would like a drop down box with 5 different names to choose from that will decide who to email the form to. And the signature is coming through on a separate page than the applications, I need it to show just like the attached forms to be legal.
