FIX SIMPLE PHP QUERY FOR MARKETING - PLEASE READ BEFORE BIDDING

I have created a simple PHP script that runs as a widget in Wordpress. This widget is a drop down of only two fields. It then sends those two fields to a page. PROBLEM: It runs 1 time then quits. When you try to run the script a second time it sends a 404 page error, page cannot be found. YOUR JOB: 1. You will clean the CSS file. You will remove anything that does not belong or is being called in order to trim down the file. The css file is presently very small, only 3k 2. You will fix the script so that it works in Wordpress. a: I have already created the script. You just need to make it work. b: I have already created the css file. You need to clean it up. c: I have already created the two wordpress template files (so PHP in wordpress is not an issue) 3. YOU WILL TEST before asking me to check. 4. You will have this completed within 2 hours. Again, everything is written - you simply need to repair the script and clean the css file of junk. 5. You will also help me get this loaded in Slider Revolution slider. <<< this is not the problem, this is simply a place I want it to load. Presently the script is in a sidebar widget. What this Script Does: A user will select the YEAR and MAKE of a vehicle and submit details. It then sends them to a page where I have marketing material. It does not store info. Its a simple drop down selection of vehicle YEAR and MAKE. <?php echo $_GET['year']; ?> <?php echo $_GET['make']; ?> There is a checkbox that allows them to select whether their car failed a test. If they check that box it sends them to a different page. Still only using : <?php echo $_GET['year']; ?> <?php echo $_GET['make']; ?> Again, this is simple, but 2 other coders lied about their skills so I have been trying to piece this together. My 3rd attempt at hiring a pro. Please ask any questions.
