I need a webpage built from a subdomain of my current website. Need a webpage where customers answer 5-7 questions, enter email and then the results from deciphered excel sheet are displayed and emailed to the user. The excel sheet will be updated daily. I would like this done as soon as possible. There is sample on my page of what the questionnaire will look like: [login to view URL] If you do good work there will be another project similar to this the following month. Please do not bid the project and then tell me it will be more expensive to complete project.
