Customize and run Unsubscribe Page (php+html)

I'm looking to make an unsubscribe page. I have a php and HTML code (see attached file ) but i cant get it work and I want someone customize that with my MySQL, so that working!
Here's the steps I'd like to have happen:
Users hit unsubscribe link in HTML email
Users either enter email in text field on page or simply hit submit button under a
text field pre-filled with their email address!
Users emali is deleted from list

its a pretty simple one page i took a look at the code and have suggestions, especially wrt security and sql injection i can create a single page unsubscribe using twitter bootstrap as base
