Create script to move data from CSV to value-name-pairs

We are looking to take 300 rows of data in a comma delimited CSV file and move the data into a flat file in the following format. The CSV column names are different from the "name" in "Value-name-pairs" below that are meant to be the output. We will give you a matching list.
