Create a WordPress Template

I have two forms that i would like to be designed / created for my website. The form has a number of fields, some text questiones and some multiple choice. I do not need any validation on these forms. There are roughly 25 fields. I have created this form myself in contact form7 but it looked very basic. I would like some element of structure / style to this form but nothing too fancy. Ideally i would like two pages. Once with the personal information form. Once this form has been completed, on the thank you page a link to the next page with the seccond form. When each of these forms have been filled out and sent i would like the subject heading of the email to be the name of the form followed by the clients name. Many Thanks
