Cost calculator setup in Wordpress.

I require a cost calculator setting up for a wordpress site. It is quite a simple one and i think I have a plugin for it. ([login to view URL]) I have attached a sketch of the required the parameters required. Although we are still waiting for the costs which I will . So for example the first question will be Are you Camping or Touring If they select touring for example it will ask them if it is a Motorhome/Campervan or a Caravan (there are different rates for these) Then it will ask if an EHU is required.
