Codeigniter: Form submission with database injection

We would like to add a new feature into our existing social network site. It is written in codeigniter. The new feature is a submission to report violations through a web form. The form will consist of 2 pages: Page 1 fields: - Type of violation (select from 2 options) - Urgency (select from 3 options) - Location (City) where the violation was seen (using google API) - Continue button (will take the user to next page) Page 2 fields: - check box "Other witnesses?" - other witnesses that have seen the violation (text area - only show if check box is checked) - Exact address (using google API and option to adjust pin on the map) - Description of the violation (text area) - Details of the user that is submitting the report (show logged in user details) - Checkbox to accept terms and conditions - Checkpox to accept other conditions - Submit button The form must run a form validation, all fields are required except other witnesses. On submission, the data must be injected into a table on the DB and an email will be sent to the user and to the site admin. Current design must be used to complete the form.
