Add FB share/like buttons to my site

I tried to add FB share/like buttons above and below an embedded video, but it does not look like I want. I want you to make the buttons look EXACTLY like this website: [login to view URL]
Requirements:
1) TOP button should be a shortcode so I can insert it myself into posts. It should not be displayed if I do not input the shortcode. Should open new window when you click the share part.
2) Bottom button should be at the bottom of every post. Remove the Prev/next buttons and make the button span the entire column. Open new window when clicked.
3) remove prev/next buttons from top of page.
