Create Wordpress Plugin

Hi i need some sort of wordpress plugin that on a certain page will allow the user to enter there details such as

All Mandatory Fields:

First Name:

Surname:

Address:

Phone Number:

Business Name:

ABN Number:

-------------------------

About your future website:

Domain Name:

Qty of Web Pages:

Qty of Images:

Do you have a logo or website banner?

Are you able to organise your own data including information, images, and logos regarding your website?

Please Upload your Attachments such as logos, images, and text

Do you wish to purchase a Domain name or Hosting account? - Link to other website if NO

Do you wish to submit your website to google for ranking?

How did you hear about us? Drop down (Word of Mouth, Website, Google, Newsletter)

(Agree to terms and conditions listed) - Click Buttons to agree or disagree

You will be contacted shortly with a quote for your website and you agree to our terms and conditions

This information will then need to be sent through server side code as a pdf with the business name on it stating that the following person and company name which has been entered in the form have agreed to the terms and conditions of our services.

I would also like an admin section so that all clients that have registered have there details stored in there including the agreement pdf

Possibly i would like to in the future connect it to a payment gateway as well
