Need Static Page Built On 2 of My Blogs - same page - With some customization

I need a simple static page created for 2 of my blogs. We will create the page and duplicate it on both sites. The page will be positioned above the actual blog, so the articles and links will be below it. The page will have a headline, a video (with a flowplayer that you will have to install), and an opt-in box to the right of the video... I will supply the raw code to the opt-in box and I'll need you to make it look better. I will show you how I want the outside to look. You just need to recreate it I will want instructions on how to change the headline and subhead from the file manager inside the host, as I will want to test different headlines on my own after you are finished. Not too difficult but I need it completed today! Send an example, be qualified with good ratings, and lets get started. Thanks
