Simple Product Finder

I have $150 for a small project, money is? already escrowed.? I need a simple product finder that will be iframed into a website. A simple database.? User would input a zip code and closest 5 places would show up to where it is sold (Business name, address, phone) - A link from the address to a google map would be fine. The map would not have to be in the request page, but would be nice.?

If you have done? this project already, this could be a quick $150. No graphics needed. I will not be waiting/looking for? bids, so essentially first legit bid request for $150 I am going with.
