HTML Template Production + Script (PHP or Perl) PSD file provided

This is a very URGENT small project. Ideally done for next week. It's for a homepage + inside template creation based on photoshop file. The template will need to be integrated with our in house CMS (easy). We will provide a layered PSD file with the design, jpg of the approved layout. One script (perl or PHP) will need to be created to managed the team page based on a MySql database (less than 20 team members). Our CMS provides the database management tools for the backend, but we need a simple script to layout the data (photo, name, position, email, phone number). Expert skills in cross browser HTML, CSS and JavaScript are essential. Programation skills like Perl, PHP and mysql are also needed for this project. We would expect a knowledgeable person to work with us, make appropriate suggestions in terms of design and implementation, and be proactive. We see this developing into a long term relationship. We are looking for someone with impeccable verifiable references. Please provide detailed resumes/bio/portfolio and samples of previous work. Thank you for taking the time to evaluate this project.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
