Real Simple OS Commerce Mod Needed !

This should be really easy. We need to have some where in the cart or checkout process some coding to identify/ look for one word in the products and be able to offer one addition attribute and prompt the user with a drop down for it. But only offer that attribute one time for unlimited amount of items that contain that word. Otherwise the user would have that attribute amount added per item, which we do not want. Again, there will be only one keyword to look for, total , in this project. Example Checkout : Blue Shirt 10.95 Red Shirt 13.95 Green Shirt 11.95 Example Prompt: 'We noticed that you ordered a shirt, would you like 2nd day shipping on this ?' And then have a drop down (YES (+40) , NO). We would need that placed somewhere in checkout. *** This attribute will only be added to the total price, you will NOT have to do any coding for shipping Mods, etc. Should be really simple. Looking to have this done today. Thanks for looking :)
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
