Edit: This page should be done in CGI and will live on a Linux server with Apache. This will be based on a browsers interface (firefox / netscape compatible) where a user requests a combination of three different types of access. The user will also have an option to choose if they have supervisor authorization or not. If yes, it will proceed to the next paragraph. If no, it will send an email to the supervisor who can approve the request. The supervisor should be able to just respond with "approve" or "deny" to move onto the next paragraph.
This request will need to append to a database (for history) and then an email will be sent to the managing user who will be given a link to another similar page to approve or deny these requests. There should be a text field to allow the managing user to add notes to his approval or explain his denial.
After approval / denial has been done, it will append to the database. The managing user should be able to access a page that accesses the database. It would give them a history of when the requests took place, when it was approved/denied by the supervisor and when it was approved/denied by the managing user.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

Thank you for your bids. After reviewing all bids, I would like to add some more information - the pages would be best done as cgi, so I believe Perl would necessary.
I have an Informix database that has all user account information - you do not need to deal with this, but please allow for a code maintainer to be able to add this information in.
If there are any more questions, please let me know as well as if this new information will alter your bids.

Thank you all for your questions. Unfortunately, it seems that my request is a bit difficult for me to relate properly.
I do have a copy of finished code from another project, that, although different is similar enough to understand database needs and how it will integrate with my system.
The filesize is 21K and is a .pl file that can be opened in any (i think) text editor. Should you want clarification and would like to see the code, please contact me with your email address and I can send it to you.
