I have a simple form [url removed, login to view] that requires the following:
1) Add feature to validate email address (email1 / email2) in the form before users submit.
2) Add a feature to upload files to a directory. Our server is using ASPupload, so you can use the demo code they provide to enable this form to upload files using this component. We want only ZIP files. If you are familiar with this component, you do not need the demo code.
3) We need to integrate a menu tree where users can make one or more selections. The menu tree will be hidden first, but can be expanded as the user clics on the + sign. See a picture of the fields

4) We need the form results to be sent to us by email upon submittion
5) We need the form results to be sent to the user (in a slightly different format) along with some text we have written. This is some kind of confirmation email and a thank you note for the user.
6) The form results should be stored in an Access Database. We should be able to edit the entries and have a huge field for comments in the edit page.
7) We need the edit page to have the following checks:
Approved /
Disapproved /
Pending
and we need to query by those fields. Example, when we edit the page, we would like to see who has been aproved / disapproved / pending.
## Deliverables
The form has been setup already and we are working on a confirmation page. You need to figure out how to send the email to both users (admin and user) as well as to integrate menu tree with the options you see.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
