Opencart 2.0 module required

We have upgraded to the new opencart v 2.0.2.0 which works well but owing to us wanting our customers to register newly purchased electrical items and serial numbers on our site, we need a modification to the My account area. We would want this as a module/extension add on, with no modifications to the Opencart core files. In the my account area we would like to add a page or pages, that will allow us to add fields to the pages, in order to collect information from customer input to register newly purchased electrical [login to view URL] are using the opencart "custom field" option in the customers tab. I guess we want the ability to add and title forms in the customer area and then have them neatly and visibly stored under a tab along with the other "my account" tabs/pages on the right of the screen. . Is anyone aware of a module that would do this or is it a module that someone could come up with for us. Must be compatible with Open Cart 2.0.2.0 Thanks
