Exim sending limits not working

Hi, I have a modified version of exim that throttle emails based on ESP (limits), it used to work very well but now for some reason stopped working as it should, I need you to find the problem, I'll give you access to this server and you should be able to detect the problem, fix it and tell me step by step how to fix it by myself in case we get it again on other server.
