Full legal name of the processing vendor
Full address of the processing vendor where functions are performed.
NMLS ID #
Date of formation/incorporation
State of formation/incorporation
Please provide the citation of the applicable statute and/or regulation which permits the assessment
and collection of such fee from an applicant/borrower by a mortgage broker.
Certification of any ownership, partial or full, of the processing vendor by the mortgage broker.
