1 Page Simple Dashboard UI Design and Coding CSS

I am looking for a CSS coder and designer to redesign the attached screenshot and code it out in CSS for me. It is a tracker where I am able to track different attributes monthly and save them. The checkmarks and x's are placeholders. In place of them, I would like to be able to have a percentage scrollbar or round scrollbar (similar to the attached images) that changes color as you scroll it. It turns green at 90%. It's yellow from 50-90%, and it's red below 50. There is also a div popup to enter some numbers in a simple form. I will be coding the back end, but I need someone to give it a good design overhaul, allow me to make some changes, then you will code it out in CSS. The deliverable to me with be the page coded out in CSS.
