Create a Form to submit info into Access DB

I'm looking to create a webpage submission form (or paper asset management db) that is able to translate information from a pdf document and input it into an access db, from which the information could then be recovered, and a copy of all files could be downloaded. Also, one could submit additional information or supporting documentation (MS Word documents, for instance) to that particular file. The ability to access MS Outlook for reminder emails, for example, would also be a requirement.
This mini-site will have rigid requirements and must be ready to go within a week from the moment all requirements are discussed.
Please have a strong familiarity in VB basic and moving information to and from an MS Access DB, and using the MS Office environment.
