Simple, single landing php landing page (one page website)

## Deliverables

Will look exactly like the text document I've attached as far as font, colors, and font sizing.

Single landing page (One page website) with three boxes for names and optin e-mail addresses. I've attached a word document with a copy of the requested landing page.

The boxes for the names and e-mail address will appear in three different locations throught out the page. The desired locations for the boxes are shown on the copy of the landing page which can be found in the attached word document.

Each box will consist of two options/lines; 1 line for the names, 1 line for the e-mail address, and a “submit?? button.

All e-mail addresses collected will be automatically saved, and uploaded to an excel;comma delimited sheet located in “root?? with a php script.

Please be advised the script needs to done before tomorrow, tops five hours after bid has been accepted.
