Annapolis Wedding Minister - develop template/images

I need a professional to create a Dreamweaver MX (or CS3) template design, with editable/non-editable areas. Need an improved banner (see webs mentioned below as examples), and other graphics for that template. One working page must be included: Email form, so that the potential customer can fill out a form and have it emailed to to us without the customer needing mail client on their computer. (i.e. not-mailto) Want to pick up the pieces myself after a professional gets it on solid ground. I'm experienced, but no expert. If the .dwt is done correctly, I'll have no problems tweeking to meet wife's needs. Will want to use similar "consistent" graphics for blog to be established on blogspot.
