Create a Wordpress page template

Hi! I need someone to create a custom wordpress page template and to identify the location of all the language files in the theme so I can translate them. The custom page template should include these fields: Product name, Quantity, choose product attribute, customer name, customer e-mail, customer phone, customer address, info about payment options, submit button When customer clicks submit, the information in the form should be sent by e-mail to the site admin, and a order confirmation mail should be sent to the customer e-mail.
