Build a Website with form

I'd like a one page website form for a manager to select hardware and software needed for a new employee. This form upon submittal would generate an email directed to a specific email address with the results of the form. This form will have checkboxes and prices along with the name of the item (See document) in order to selectively choose certain items and should tally the costs at the bottom. I'd like a SQL database to manage the content so I can update pricing and add new offerings as well as to store data from past submitted orders. This form should also generate a unique number for the order. I can provide RDP access to an AWS instance with Server 2008 and SQL 2008 R2 Express reside for development.
