Create Online Test

I need on online test created using php and mysql and installed on my servers. Here’s what I need: • Test with 4 possible answers to choose from • 150 questions • Specific user login id and password which automatically ends once the user passed the test. • User should have the ability to take the test as many times as possible until passed • Each time the user takes the test, answers should be randomized • Completed test must be emailed to 3 designated sources • Admin interface to modify questions. • 15 Questions per page Please provide project price quote and estimated timeline. I need this completed as soon as possible.
