Calculation Form

What I need is an EASILY customizable form. This is for a restaurant. I need to be able to use this form on more than one item... The form will be used in this context. Customers click on the package: They type in the amount of people they are going to have: A certain price will be set per person: Everything on this is customizable... Then they will be directed to a Paypal payment page where the price that was set in the calculation form, will be that set price... When they fill out all of the information on the form and it is calculated, and the payment has been sent **number of people, amount of certain things, etc** it will all be sent to two emails provided, both the form with ALL of the information collected, and the payment receipt... The form will NOT be sent unless the payment is... This is VERY important! And Like I said, I need it to be EASILY customizable so I can add it to Any item we have to sell...
