Digital Tablet Form Software

What I want is a coder who can make my 1 page work order form appear in the software the same as it is on paper. Then I want the handwriting input from the digital tablet to appear in the the fields on the screen so it can be emailed etc. The output formats of .pdf are already included in the software, but I would like to get them in a database format as well for the next phase of the project. Attached you will find the link for downloading the software and an example of the basic form. I also want some help finalizing my 1 page form in the correct format.
