Custom Search Tool with Image and Advanced Search

We need a custom search tool that 1) will be integrated to function with an image and 2) will provide an advanced search option. The tool should be easy to navigate and fully functional without any glitches or errors. The basic search setup should consist of two fields: First field: Subject Second field: City, State User should be able to come in and type in a search term/subject, select the area that they want to search in and hit a "Search" button. An image will appear to show them their available options. A text-only link for "Advanced Search" should be visible. The "Advanced Search" option should open and consist of several additional fields. Users will put in their details and click an "Advanced Search" button. Again, the image will appear to show them their available options.
