Creating an Doc template (Tech Tickets)

I would like this project in Excel format. I have an template that needs improvement. I need it more colorful and I would like to place a company logo. I would when a user adds the information and when their done with the 1st record of the template, and if they need to add a 2nd record, they can press a button so the next template record can go underneath the original record. I would like someone who is detailed oriented to take care of this project. The following fields in the template are... Current Date Date/Time Required Ticket Resolution Incident Resolution Ticket Number I also would like exceptions, meaning the current date cannot be modified, and all fields required. I also would like a button so it can create an send mail() function for an easy email feature. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
