Creating forms on Word 2002

I am trying to learn how to create a new medical form template using Word. The form will include underlined blanks for completion by the user (Name, Address, etc.), yes-or-no boxes (First time patient?, etc.), a list of checkboxes (various symptoms, etc.) and empty text boxes (to describe symptoms, etc.) with dropdown boxes for easy completion. In particular, I want to learn how to create dropdown boxes and how to populate the drop down boxes with choices so that the person filling in the form can quickly complete the textboxes or underlined blanks by clicking on a dropdown arrow and highlighting his or her choice. The choices within the dropdown boxes should include the option for the user to fill in the textbox or blank line in lieu of choosing one of the choices provided in the dropdown box. I also want to know how to create empty underlined blanks within the form so that the person completing the form can fill in the blanks without any horizontal shifting within the underlined section. The person would either type in the missing information or choose from a drop down box. I also want to create check boxes for easy checking or un-checking. I tried to figure out how to accomplish the above from the Help section within Word with no success. I managed to create checkboxes in Excel and then copy them over to Word, but this seemed unsatisfactory and clumsy. What I want from a bidder is a precise step-by-step guide for each element. In other words, the bidder must detail every (and I mean every) click of the mouse and every action that one needs to take to accomplish each element. I also want an explanation on how the create and size the textboxes and align the boxes and underlined blanks so that the form's alignment is justified on both the left and right sides.
