Code Me A Contact Form and Short Survey (aybform)

**Overview of features required:** ? Coding a contact form to send an email ? Putting a name and email into a MailChimp list ? Some basic PHP knowledge ? Familiarity with Wordpress ? The ability to code a basic 5 question multi-choice survey I have a Wordpress site and I've created an entirely different landing page to promote our book. I've created these in the theme file and that works great. It has a contact form on the right as you can see: <[login to view URL]> I'd like the book picture and the form to "stick" as the user scrolls down the page, so when they get to the bottom, it's still there. I need "live" validation on the fields using TipTip or something similar. Phone number and city are the only optional fields. When they click in the box to type, it should remove the default text. If they click off either without typing anything, or typing, then deleting - it should put the default text back. Whatever they do, if there's no text in the box, it should have the default. When you install this validation, please advice and check with me on where to install it on the [login to view URL] domain. We (stupidly) have some images files which are hosted on a different domain ([login to view URL]). I'd like all of this put somewhere sensible on advantixgroup.com. Lets discuss this. Clicking the 'Send me my book' button will take them to a 'questions' template I've created. (which we will) When they complete the survey, it should send an email to <ads@[login to view URL]>and <sabrina@[login to view URL]>. It should also put them into a MailChimp list. We have the API documentation for this and 2 examples of it working on another site of ours. If they haven't answered all the questions. Can you use TipTip to put a nice red bit of validation next to the question with the text "You need to select the answer that best fits your situation" (All 5 questions are required) Once complete, it needs to send them to: <[login to view URL]> It should also send a plain text email.
