Custom form for reoccurring payment through Stripe

We are receiving payment through the Stripe payment gateway ([login to view URL]). So far we have accepted payments through a form set up and integrated with Wufoo, but as we are now looking to create reoccurring payments, we need a custom form. Form should allow for custom fields, amount should be set by customer/client, as well as the ability to choose whether amount is a monthly payment or an annual event. - First milestone is working prototype of payment form. - Second milestone is fully working form, with Podio integration ([login to view URL]) so all transactions are pushed to their database through API.
