Custom Guest List Application in php/mysql

I'd like a guestlist feature on my website.
1. There are already "groups" which have ~8 fields. Groups would need to have extra fields added, 5 for each day of the week. Admins (who already can edit groups) can enable guestlist for a day of the week (or many) and enter email fields (comma separated) that are private (admin use only) along with the other public fields.
2. There will be a new guestlist page with a form where users will go to apply for each group's guestlist. Users choose date from dropdown, then the next field is populated with groups that had guestlist enabled on that date (from step 1). User enters name, email, etc. (registration not required). User is taken to confirmation page where they can fb like/tweet their info. Users are not allowed to fill out the form after 9pm for a date on the sameday. All fields are required.
3. Admins can see all guestlist entries by date by group. A table is displayed in admin panel that shows all groups under a date that have entries. When clicking on a group under a date, new page is loaded with a new table displaying all entries. Admins approve or reject entries. Once approved or rejected, the user is notified via email. Admins can add extra entries from the admin panel under each group.
4. Admins can export a csv of the table which has columns for each field the users enter in the guestlist form. These csvs are by group only. Finally, a csv is auto-emailed to the emails provided in step 1 at 9pm everyday. Admins can add new entries from the admin panel.
Design (css) is not required. I do front-end work so css and design is not needed, please let this reflect in your bid. Knowledge of pligg is a bonus but not required.
The site uses the CMS pligg. Which uses smarty for templating and is php/mysql.
I can provide a more detailed spec via pmb a well as the functioning site.
Please note, that NO design skills are required.
I will do the design & css/html as long as the smarty file is setup properly for the page.
