Ajax Form

I'm looking for someone who can generate an Ajax Form: The input of the form consists of: arrival date, month and year, number of nights. (form can be provided) After having entered the submit button the form needs to disappear, a GIF loading indicator needs to appear and the entered data needs to be printed on the screen.
