Convert HTM questionnaire into flash app + pass variables to SQL db when done

Task is pretty straightforward: Convert this HTML questionnaire <[login to view URL]> into a flash questionnaire in the style of <[login to view URL]> , and then run a logic tree at the end to distill out a product code. This entire app exists just to distill the product code. Note there will be some very simple math used in the logic to define the rules, notably calculating LTV (Mortgage Balance divided by Value/purchase price), payments (both interest only and amortized), closing costs (simple calc like $2000 + 2% of loan amount), etc. Amortized loan calculations are surely the toughest part. Once the product code is decided then a few other codes will need to be passed to the application, or that can be handled on the app side w. php/sql. Below are examples of the logic defining which program is chosen, the rest has no suprises and is more of the same.
