We are looking to have a database developed using MySQL and PHP that will be accessible over the internet. The database will need a section for people to add information to the database, we have attached a copy in excel of what we are looking for. We will need a page that provides our visitors the ability to search by city/town, province/state or country and will display the records below on a web page, we would prefer a vertical display with the ability to scroll through it based on returned results. Returned results should only display organizational details and not personal details. We would like users who have supplied a user name and password the opportunity to go back in and modify or delete their listing. We would also like a method where we can go in and modify or delete a listing. i.e. works with a master username/password. We will need all necessary html coding that will need to appear within the website.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
## Platform
Our IP providers are using MySQL 4.1.2.2 and PHP 4.4.7 and will need to be viewable by Firefox, Netscape and Internet Explorer.
