Been ripped off, need someone to sort out my contact form

Thanks to a provider on here I've wasted a day and been highly embarassed showing his work to my client - I can only assume he didnt like the genuine review I left him and he uploaded edited work once he'd sent me the supposedly correct work. So, as a result I am going to be out of pocket - again - in order to get something really simple working. 1. Contact form works in a sliding div on my clients site - the CSS is poor and needs edited 2. The contact form is SUPPOSED to send the contents of the form to three email addresses, one To, one CC one BCC it doesnt send - it is also supposed to appear to be from the person who submitted the form but I've no way to check this as the emails dont come through! 3. The form is supposed to validate contents - to ensure required fields (all of them) are completed - it doesnt (not the last providers fault that was missed the first time I posted this project - did I mention that, this is the THIRD time I've tried to get this sorted hence my near anger and extreme frustration). 4. When the form is submitted the form is supposed to be replaced by a message to advise that the message / request has been sent correctly. So, who can actually do this for me, within a few hours at most? I need this done today as I have to meet with my clients tomorrow to explain why it wasnt working when they were asked by me to test it. That'll be an interesting meeting for sure!
