Create form which collects name/email and then allows access from backend

We currently have a system where we setup a page on our server with some code that has a client enter their name and email address and they are then able to gain access to our PDF newsletter. In this respect, we really don't care if they enter bogus info, etc. (provided its a valid email format). It's a very simple process.

When they submit their data, it drops a cookie on their computer that allows them to access the pdf. At the same time, it writes a record to a file on the server with their information. Then .. every few days, I collect the info from the server and email it to MY client (major pain to do this)

This has been working fine but its time to try and make this easier AND more streamlined. So, here is what I want to have done.

** first of all, this has NOTHING to do with sending out newsletters - we do that all from the office - and systems like constantcontact don't meet our needs **

Create a simple backend where I can enter some text, the date, the name of the newsletter, upload the PDF.

This would create a link like this: <[url removed, login to view]> (just an example)

We could then send this link out to the clients, they would enter their information and it would give them access to view the PDF.

It would also write a record back to some type of database or file.

Then, we would be able to give our internal users password protected access to VIEW the file that shows who has entered their information etc.

I can show you an example of how we currently do it and it actually works perfect (besides the part where we provide password protection to the end data).
