ASP.NET VB.NET Leads capture page

Create a simple 2 step lead capture form. No design work needed. We will combine what we receive with our VS2005 project. What you will be designing is a leads capture site for real estate buyer leads. On page 1 allow the user to select a state. The state will then query the database and pull all the cities. The user then selects his city, min beds, min baths, and price range. Price range should be comprised of two drop down list for min and max. prices should be in $50k increments starting at $0 - $1,000,000+. Page 2 will then display all distinct cities within a 20 mile radius of the one they selected in the first step. I already have a database and stored proc that returns this. It needs to be slightly modified so that only distinct cities are returned since it now also includes zipcodes. User then checks off the additional cities, enters a bit more search criteria and some contact information. They submit and the data is saved to the database and an email sent with the search criteria and contact information to us. Email and other settings should go in the web.config. Please see attached zip file which contains a word doc with a sample layout. NOTE: The first part of the sample is not exactly as I described in our requirements. What you develop should follow what I outlined above. Buyer will receive exclusive and complete copyrights to all work purchased.
